What is the Support at Home program?

Support at Home is a government in-home aged care funding program that’s designed to help older Australians stay independent, healthy and connected to their communities, while receiving the right support in their own homes.
From 1 November 2025, it replaced Home Care Packages and Short-Term Restorative Care. The Commonwealth Home Support Programme (CHSP) will continue separately for now and is expected to transition no earlier than 1 July 2027.
How does Support at Home differ from previous Home Care packages?
The program is designed to give you greater control over your funding and how it is spent on services, empowering you to make informed decisions about your care.
Support at Home focuses on:
improved choice and flexibility in services
streamlined access and assessment
better coordination and integration of care
greater transparency and control over how funding is used
What services can be included?
Support at Home services are categorised into either:
ongoing support
short-term support
Ongoing support
These services are grouped into three categories:
Clinical supports: Clinical support focuses on health, wellbeing, and maintaining or improving physical function. Services may include:
Nursing care, such as wound care, medication management, health monitoring, and chronic condition support
Allied health services, including physiotherapy, occupational therapy, speech therapy, and podiatry
Nutrition support, including dietary advice and meal planning to support health and recovery
Care management, helping to coordinate services, appointments, and ongoing support needs
Restorative care management, focused on building strength, independence, and confidence following illness, injury, or hospital stays
Independence supports: Independence support helps people remain active, safe, and engaged in their everyday lives. Services may include:
Assistance with daily living activities, such as personal care, dressing, and showering
Support with mobility, including walking assistance, transfers, and the use of mobility aids
Help to stay socially connected through transport assistance to attend appointments, community activities, or social outings
Carer respite services, providing short-term relief and support for family members and carers
Everyday living supports: Everyday living support assists with routine tasks around the home to make day-to-day life easier and safer. Services may include:
Help with household cleaning, laundry, and general home maintenance
Gardening and outdoor maintenance to keep the home environment safe and accessible
Assistance with shopping, errands, and essential appointments
Meal support, including meal preparation, delivery, and assistance with eating when required
Home care through Calvary can include supports such as:
clinical and nursing care
personal care and social support
transport
help with domestic tasks
equipment and assistive technology
nutrition planning and food preparation
Support at Home funding levels (classifications) for ongoing services
If you’re eligible, you’ll be assigned one of eight ongoing funding classifications:

Short-term supports
You may also be eligible for short-term supports through three short-term care pathways:
Assistive Technology and Home Modifications (AT-HM) scheme: this pathway provides short-term access to equipment or small changes to your home that make everyday tasks safer and easier, such as mobility aids or bathroom modifications. It helps reduce the risk of falls and supports you to remain independent at home.
Restorative Care Pathway: this pathway provides short-term, goal-focused support to help you regain strength, mobility, and confidence after illness, injury, or a decline in function, often through therapies such as physiotherapy or occupational therapy. The support focuses on rebuilding everyday skills so you can do more for yourself and reduce the need for ongoing care where possible.
End-of-Life Pathway: this pathway provides short-term support for people with a life-limiting illness by helping manage comfort, symptoms, and daily care needs at home, in line with the person’s wishes. It also offers practical and emotional support to families and carers, adjusting care as needs change.

How can I access Support at Home?
Once you’re ready to make the first step,
Register with My Aged Care: The first step is to register with My Aged Care, the Australian Government’s entry point for aged care services. This registration allows you to request an aged care assessment and start the process of accessing Support at Home.
Complete an aged care assessment: Once registered, you’ll be assessed through the Government’s Single Assessment System. This assessment determines the level of Support at Home funding you may be eligible for, and the types of services you can access from the service list.
Receive your assessment results: My Aged Care will notify you of the outcome and information regarding your approved services.
