Support at Home program

The Support at Home program is a Australian Government initiative designed to help older Australians live independently in the comfort of their own home for as long as possible.
Whether you need a little extra help around the house or more regular support with daily activities, this program connects you with the care and services that are right for you, so you can continue doing the things you love.
On 1 November 2025, Home Care Packages and Short-Term Restorative Care programs were replaced by the Support at Home program. The Commonwealth Home Support Programme (CHSP) continue to operate separately, with plans to transition into the new program no earlier than 1 July 2027.

What the Support at Home program offers:

Support at Home is designed to help you remain in familiar surroundings for as long as safely possible. Your home, your routines and your independence matter.

From help with personal care to nursing, allied health and social support – the program covers a broad range of services to keep you healthy and well.
Support for the whole family
We understand that caring for a loved one is a team effort. The program also offers respite support for family members and carers, so everyone feels supported.
You're in control
You have a say in how your care is delivered and who delivers it. The program puts you at the centre of every decision, giving you and your family peace of mind.
Personalised care, tailored to you
No two people are the same, so neither is your care or goal plan. Together with your care team, you'll receive services that reflect your individual needs, goals, and preferences.
Regular reviews to keep up with your needs
Your needs may change over time, and your care or support plan can too. Regular check-ins ensure your care continues to match what you need most right now.

What to expect

Getting started

Initial consultation
A member of our Calvary Home Care team will work together with you and your significant others to design and manage a personalised Support at Home program to reflect your individual values, goals and unique requirements.
They will set up services, source useful products, make referrals and monitor your progress and budget. Experts in community services, they will also advocate on your behalf so your specific requests are met.
Single Assessment System
The Single Assessment System will determine the level of funding you are eligible to receive for your care. This will also prescribe the services you can access from the service list.
Once you have been approved for the Support at Home program you can then choose Calvary as your preferred provider.
Different levels of Support at Home funding
If you’re eligible for Support at Home services, you’ll be assigned a classification levels.
Classifications cover:
  • ongoing services with 8 different funding classifications
  • short-term services with 3 short-term classifications:
  • the Assistive Technology and Home Modifications (AT-HM) scheme
  • the Restorative Care Pathway
  • the End-of-Life Pathway.
Each classification level determines your quarterly funding amounts to access ongoing services like clinical care, independence support, and everyday living assistance. These funding amounts are inclusive of care management funding.

*Quarterly budgets and annual amounts are effective from 1 November 2025 and are subject to change in March and September each year in line with indexation.
Short-term service classifications include three care pathways:
  • the Restorative Care Pathway,
  • the End-of-Life Pathway, and the
  • Assistive Technology and Home Modifications (AT-HM) scheme.
Funding amounts are indicative and are subject to indexation revisions. Funding amounts for each classification are outlined in the Schedule of Subsidies and Supplements for Aged Care.

The Support Home service list has two categories.
Clinical Supports: Nursing care, Allied health and other therapeutic services, Nutrition, Care management, restorative care management
Independence Support: Support delivered to help you manage activities of daily living and live more independently. Services are flexible, and you can work with your provider to adjust them as your needs change.

Budgets are based on your individual aged care assessment, which considers your needs, goals, and health status. Funding is provided quarterly, giving you flexibility to plan and adjust your services. When you receive your Support at Home funding package, the amount you contribute depends on two factors: your personal income and assets, and the specific services you select.

Your package fully covers all clinical services you require, giving you peace of mind about your health support.
You maintain control of your daily support costs.
For Independence and Daily Living supports that help you maintain your lifestyle, Services Australia will determine what your contribution will be to the cost—keeping quality care affordable while giving you choice in how your care dollars are spent. Calvary will work with you to design a package that supports your needs and affordability.
