Calvary Health Care

Calvary acknowledges the Traditional Custodians and Owners of the lands on which all our services operate. We recognise that these Custodians have walked upon and cared for these lands for thousands of years. We acknowledge the continued deep spiritual attachment and relationship of First Nations peoples to this country and commit ourselves to the ongoing journey of Reconciliation.

Advance Care Planning

Advance Care Planning is the process of planning and documenting your future health care wishes.
Other important planning such as wills, financial matters and living arrangements can also be useful.
Your health care team can help you and your family think about and plan for all stages of your care.

There are a number of guides that can help you figure out what is important to you as you health changes:
  • Dying To Talk
  • MND Vic End of Life Guide
  • The Manual For Living and Dying in Style
Reading books, watching movies and attending community conversations can also help your think about what matters to you.

Each State and Territory have their own legal document.
Below are the links to the Victorian documents:
  • Take Control Booklet (Medical Treatment Decision Maker/Advance Care Plan)
  • Advance Care Planning
  • Financial Power of Attorney
  • Last Will and Testament
  • Information for people without decision-making capacity

Our preferences and wishes can change as we learn new things or have new experiences. It is important to keep reviewing what matters to you as your health changes.
