The Integrated Model of Care (The Calvary ‘Eco-system’)
Calvary’s integrated model of care means that we are able to provide uninterrupted high-quality care through all funding programs. We understand that as people gradually transition through different stages of their lives, their needs also change. From private home support, Commonwealth Home Support Programs to Respite or Home Care Package programs. During this process, we will have an expert caring for them at every step of the way. We also provide independent living and residential options if conveniently located near you. Save your time providing excessive amounts of paperwork while dealing with multiple service providers during these life changes. We’ll make the transition easy and simple. Being a part of the Calvary network means that we are a seamless ‘one-stop shop’ aligned with the same vision and values.
